The Five Behaviors of a Cohesive Team profile, which provides both individual and team feedback, is grounded in the model described in The Five Dysfunctions of a Team, the internationally best-selling leadership fable by Patrick Lencioni. With this program, participants will learn how, as a team, they score on the key components of the model: trust, conflict, commitment, accountability, and results. Additionally, the program is powered by Everything DiSC®, a model that helps individuals to understand themselves and others better. Using these results, participants will be able to create a better, stronger team.

The most effective way to implement and develop the strengths  of The Five Behaviors of a Cohesive Team is though a multi-day facilitated coaching and team building program that provides the training and experiential process of The Five Behaviors.  The number of days and the scope of the facilitated process will depend on the independent needs and budget of the client.

Following the training workshop, on-going follow up consultation ans coaching can be provided to strengthen the team over time and to assess and reassess the growth of the team thereby insuring that the team is in a dynamic program of growth and development..
New The Five Behaviors of a Cohesive Team™ Take-Away Cards

The Five Behaviors of a Cohesive Team™ Take-Away Cards are excellent reinforcement aids used by participants after the session to help remind them about what they have learned. The Take-Away Cards link the five behaviors with reminders of how to effectively approach each DiSC style. Each set contains 100 cards, 20 each of The Five Behaviors:

    - Building Trust Among One Another
    - Healthy Behaviors of Constructive  Conflict
    - Clarity and Buy-In To a Commitment to Decisions
    - Guidelines for Holding One Another Accountable
    - Focus on Achieving Collective Results



One set of cards are included in the Facilitation Kit (enough for 20 participants).
The Five Behaviors of a Cohesive Team Facilitation Kit

The Five Behaviors of a Cohesive Team Facilitation Kit includes everything you need to successfully facilitate The Five Behaviors of a Cohesive Team sessions.

The Facilitation Kit includes a USB drive with facilitation and support materials and a set of takeaway cards for use by the participants. The Facilitator’s Guide, PowerPoint® presentations, and participant handouts can be used as is or customized for your needs.

The program consists of seven modules from 1 hour to 3-1⁄2 hours in length, plus a Getting Started section. Each module can be found in the Facilitation Materials folder on the USB drive and includes:

        - Facilitator's Guide in MS Word
        - Participant handouts in MS Word
        - PowerPoint® with embedded videos

In the facilitation, each module builds on the previous one, helping participants see how each behavior serves as a foundation for the next. Exercises within each of the modules give participants an opportunity to begin practicing the behaviors. 

Further, DiSC® styles are used as a point of reference in each of the five behavior modules, allowing participants to see how their own and their teammates’ styles affect their work as a team

Team Assessment and Profile - Includes Individual Reports for Each Team Member and One Annotated Team Report

​The Five Behaviors of a Cohesive Team™ assessment gives team members a sense of their team’s unique strengths and areas of improvement. It’s a tool that evaluates the team’s dynamic across the five fundamental behaviors so members can decide what improvements are necessary for becoming a higher performing team.

The assessment also serves as a powerful measurement for growth. By first taking the assessment at the outset of the development effort, the team will establish a baseline for the current state. Then – after the team learns and practices new skills – taking a re-assessment will provide valuable feedback about where the team is progressing and about where more focus must be placed.
The Five Behaviors of a Cohesive Team™ assessment now includes an Everything DiSC® behavior-style assessment for each team member, with the results presented on a single graph that plots the personality and behavioral “style” (Dominance, Influence, Steadiness and Conscientiousness) of everyone on the team. Personality and behavioral style is a key determinant for how people present themselves and interpret the words and actions of others. Gaining a better understanding and greater appreciation of each team member’s style gives the team as a whole the ability to work together more effectively.

Order one (1) assessment for each member of the team. The results of all assessments will be aggregated. Each team member will receive an Individual Report showing his/her DiSC personality style coupled with the team's overall assessment of itself across The Five Behaviors of a Cohesive Team (Trust, Conflict, Commitment, Accountability and Results).

Also included is one Annotated Team Report, which the team facilitator can use to help guide the discussions.

The online Five Behaviors of a Cohesive Team assessment includes two types of questions: those designed to determine DiSC style, and those designed to evaluate how frequently the team engages in the five behaviors. The responses are integrated into a single, personalized report for each participant that reflects both their individual DiSC style and the team’s composite evaluation of its performance on the five behaviors. Each person receives a personalized, individual report. 

In addition, an Annotated Team Report is included for the facilitator. This is a valuable facilitation tool that can be used in preparation as well as during the session. The Annotated Team Report is a copy of the team profile with the following notations: 

    - Expanded explanations of profile content 
    - Scoring interpretation 
    - Resources for additional information 
    - An appendix that includes a ranking of all assessment items by team average, the team’s percentile for each of the five behaviors, and       benchmark comparison data for all teams who have taken the assessment. 

The Five Behaviors of a Cohesive Team™ Profile can also be used as a standalone assessment.
